Survey On Personal Health Budgets

If you are a Personal health budget or integrated personal budget user, NHS England would like you to hear about your experiences by completing a questionnaire. In what is intended to be an annual survey, you will be asked questions about the care and support you receive and your thoughts on the processes and outcomes involved. All information will remain confidential, will only be used for the survey and will not affect the service received by any individual.

Here is the questionnaire and any comments or queries about the survey can be sent by email to England.personalhealthbudgets@nhs.net..

Anyone who needs support filling out the survey can call the helpline on 0800 783 1775 (Mon – Fri from 9am – 5pm).

NHS England is also holding an event in London on Wednesday May 9th during which personal health budget holders will meet up and celebrate the difference it has made to their lives. Details are available on the myPHBstory site.

Disability Sheffield is a forward-thinking, energetic organisation doing a lot of positive work in Sheffield. I have no doubt they will continue to do so for a long time to come.

When I moved to Sheffield nearly five years ago it was my "go to" organisation. I expect it to stay that way. All the staff and volunteers seem to pull out little gems of effort that disabled people like me couldn't do without. Professionally and personally - I love em :)
