The National Institute for Health and Care Excellence (NICE) has published new guidance regarding Older people with social care needs and multiple long-term conditions . The guidance promotes an integrated and person-centred approach to delivering effective health and social care services. Aimed at social care and health practitioners, managers and commissioners, the guideline recommends ensuring that older people with social care needs and multiple long-term conditions have a single, named care coordinator who acts as their first point of contact.
