Voluntary Action Sheffield and Co:Create are trying to understand where the experience of going to hospital and coming back home could be improved for older people in Sheffield. The purpose of this will be to develop recommendations for more involvement of voluntary organisations in the planning, discharge and support at home for patients and carers across Sheffield.
They want to know about the experience of people in Sheffield aged 55+, from their perspective or from their carer’s or family’s perspective, of being admitted to hospital and sent home again, with or without care. We also want to hear about people who are in this age group and have not recently been admitted to hospital and what is keeping them well.
This survey is anonymous and will take around 10 – 15 minutes to complete.

Please complete this survey if you live in Sheffield and:
- Are aged 55 or over
- Care for or are related to someone aged 55 or over and can comment on their experience
The survey is anonymous and and will feed into the data for the final VAS report back to Sheffield Clinical Commissioning Group ( CCG ).
