Directory Independent Living Triage

Sheffield City Council’s public information resource, the Sheffield Directory has a new ‘Triage Tool’.

It’s designed to guide you through a simple self-help assessment to identify areas where you may need support at home.

For example, if food shopping is becoming difficult, the tool links you to a range of solutions, such as:

• Online and telephone supermarket services

• Food delivery schemes

• Community transport

• Mobility aids

• Computer classes

• Home care services

The Triage Tool offers a practical alternative to contacting Adult Social Care for a formal needs assessment. It encourages people to explore support options independently, empowering choice and control.

You might find the Triage Tool especially helpful if:

• You’re exploring different sources of help and support at home

• You’re a carer wanting to learn more about available services

• You’re a support worker helping someone remain independent

It comes in the form of a questionnaire and based on your answers, you’ll receive links to useful information, services, and resources.
