CHANGE - Moving Back to the Community Questionnaire

CHANGE would like to hear about the experiences of people with learning disabilities or autism who have come out of treatment units (inpatient hospitals) and back into the community. You can view their video asking for help on Youtube .

It is very important to hear people’s views because the findings – what works well and what can be improved – will be used in Transforming Care with the Department of Health. The information will be used to tell health and social care workers what is best for people when they live in the community and CHANGE will compile a video to use in training.

To share your experiences please complete the Moving back to the community questionnaire by 30th November 2016.

    My son was sectioned twice, just to get a diagnosis of ASD PDA 6 weeks after discharge placement broke down. Home now and a new provider has started..such a ruined life..

When I moved to Sheffield nearly five years ago it was my "go to" organisation. I expect it to stay that way. All the staff and volunteers seem to pull out little gems of effort that disabled people like me couldn't do without. Professionally and personally - I love em :)
