Type 2 Diabetes Survey

Do you live in South Yorkshire, are between 16-39 years old, and have Type 2 Diabetes? Share your experience on the best way to promote information relating to Type 2 Diabetes and be in with a chance of winning £100.

NHS South Yorkshire Integrated Care Board ( ICB ) is aiming to better understand your experience of living with Type 2 Diabetes , using diabetes support services in South Yorkshire. and how best to promote their services.

Your input is very valuable and will help guide their efforts in improving the way support for people living with Type 2 Diabetes is promoted and delivered. The survey will only take around 10 to 15 minutes of your time, and your responses will remain confidential.

Everyone that completes the survey will have the opportunity to enter a prize draw for a chance to win £100 worth of shopping vouchers.
