Healthwatch Sheffield wants to hear about your experiences of being assessed for a care package. If you’ve been assessed (or reassessed) recently, please tell us:

- Does your care package meet your needs?
- What was your experience of the assessment like?
- Is there anything which could have improved your experience?

We want to collect your views so we can find out what is working well, and what could be better. We can help to influence and improve our local services by getting your comments heard by the organisations that design, run and inspect those services.

What is Healthwatch Sheffield?

Healthwatch Sheffield is your independent consumer watchdog for health and social care. We’re here to help adults, children and young people have a say in how services are designed and run.

Local Healthwatches have been set up by the government to run in every council area in England. We’re independent and our services are free to use.

When I moved to Sheffield nearly five years ago it was my "go to" organisation. I expect it to stay that way. All the staff and volunteers seem to pull out little gems of effort that disabled people like me couldn't do without. Professionally and personally - I love em :)
